As apart of my contracts with these insurance providers, Infinite Therapy & Wellness, PLLC is required to collect co-payments and/or deductible amounts at time of service.  To determine the amount of your co-pay/deductible, please check with your insurance provider.

If your insurance company is not listed as an in network provider, or you would prefer to not file a claim with your insurance company; you have the option of paying out of pocket.  Rates for out of pocket services will vary depending upon the length of therapy sessions, as well as whether it will be an individual or couples session.  In person and virtual sessions have the same rates.

If you are unable to afford the rates due to financial limitations, Infinite Therapy & Wellness may still be able to assist you. Consult with the therapist to discuss payment option.

Rates are determined by each organization's individualized assessment of their needs.

Several employers and organizations offers as apart of their wellness package free or reduced rates for individual and/or couples therapy services.  Check with your employer  to confirm if this option is available to you.  If it is, please inform at time of scheduling your EAP authorization code.

There is no charge for brief or routine phone calls.  However, more lengthy telephone calls will be charged exclusively to the patient. The charge will be determined according to the length of time involved.

Certain activities undertaken on a patient's behalf during or after treatment will be charged to the patient.  This may include, but is not limited to, Disability Determination reports, Vocational Rehabilitation reports, Department of Social Services interaction, consultations at your request with another professional, or any other request not related to your therapeutic care or continuity of care (i.e. transfer of your care to another clinician).

Record copying fees are set by General Statute §90-411

There is not a charge for appointments cancelled more than twenty-four (24) hours in advance. 

Your insurance cannot be billed for missed appointments.  For missed appointments, or any sessions cancelled less than 24 hours in advance, you will be billed a "no show" fee of $100.00.  

Consultation with the therapist about exigent or unforeseen situations may eliminate this "no show" fee.
